PHS Staffing is looking for quality Licensed Practical Nurses (LPNs) to join our ever expanding team. We just recently received our April needs request from one of our Long Term Acute Care (LTAC) facilities located in the East valley of Phoenix. We have both Day and Night shifts available. These shifts are going to fill up fast, so respond asap to insure your choice of schedule. This is great opportunity to work in a challenging yet rewarding environment and on your schedule.
Requirements:
-1 year LPN experience within the last 2 years
-Drivers License
-SS Card OR Passport OR Birth Certificate
-Professional Nursing License (AZ or Compact)
-AHA CPR (front and back with signature)
-Physician Statement (dated within the last year)
-TB Test (dated within the last year) OR Chest Xray with TB questionnaire
-MMR Records (2 different shots OR 1 titer from anytime)
-PHS Compliance Documents (ask for details)
